        As a Social Worker you will be responsible for serving as a liaison between the resident and their family and facilitating staff and the community and assisting in the planning of long-term care for each resident.
        Additional responsibilities of the Social Worker include:
        Speaking at community functions and conducting tours and educational sessions.
        Assuring successful transfer of resident both within the facility and to outside agencies.
        Assessing and developing a plan of care that meets the residents’ needs.
        Completing all documentation related to social work.
        QUALIFICATIONS:
        To qualify for the Social Worker, you must have effective communication skills and be able to work with individual of all ages.
        Additional requirements of the Social Worker include: Bachelor’s degree in Social Work or a human services field including but not limited to sociology, special education, rehabilitation counseling or psychology.
