Patients would be charged $10 monthly premiums and $100【C12】______they showed up at the emergency room with a non-emergency. The state and managed -care companies would control which services to provide to Medicaid patients. Refund rates have dropped so low that many doctors【C15】______not to treat Medicaid patients.
